Work, Practicum, and Clinical Placement Documentation Requirements

Many programs require documentation for non-academic requirements as a condition of admission or in order to participate in practicum experiences. If these appear on your To Do list in MyQuest, it means you have not completed these requirements.

Immunizations

Programs with a work, practicum, or clinical placement may require up-to-date immunization records. Please refer to the following immunization checklist (18K pdf) to determine which immunizations your program requires. 

Instructions for completion of the "Record of Immunization":

  1. Download the immunization form and instructions (424K pdf)
  2. Follow the instructions on the form.
    If you go to Alberta Health Services’ Immunization Business Unit in Edmonton to have your form completed, please take your NorQuest College acceptance letter with you to your appointment.

  3. Submit a copy of the "Record of Immunization" to NorQuest College Health Services

    • By fax to 780.644.6151

    • By mail to 10215 108 Street, Edmonton, AB T5J 1L6. Attention: NorQuest Health Services

    • In person to Room A109 in the Main Building on main campus

A nurse from NorQuest will review the form and update your "To Do" list accordingly. If all requirements are met, the Office of the Registrar will be advised and this part of your application for admission will be complete. If you are waiting for chest X-ray results or blood work results in the mail, it is your responsibiliti to submit these records to NorQuest Health Services a soon as you receive them. If you have any questions about your "To Do" list immunization requirements, contact NorQuest Health Services at 780.644.6155.

4 Steps to meet the immunization requirements

  1. Obtain records for any immunizations you have received in the past.
  • Ask your parents. If you received immunizations as a baby, your parents would have been given an immunization book.
  • If you received immunizations at NorQuest Health Services, you can call 780.644.6155 and request a copy of your records.
  • If you attended school from grades 1 to 9 in Edmonton, Sherwood Park, or St. Albert, call Alberta Health Services at 780.413.7985.
  • Check with the local public health unit closest to the school that you attended from grades 1 to 9 if you remember getting immunizations at school.
  • If you have worked as a nurse or volunteered at a hospital or nursing home, the occupational health office where you worked would keep copies of the immunization and blood work you had done there.
  • Immunize Canada has contact information for all health authorities for every province and territory in Canada. You may visit their website at immunize.ca.
  • Check with your doctor’s office if you received immunizations there.
  1. Complete your immunizations.

If you live in Edmonton and the surrounding area:

  • Make an appointment with the Alberta Health Services Immunization Business Unit at 780.735.0100 and identify yourself as a NorQuest applicant.
  • Take a copy of your past immunization record and the NorQuest immunization form and instructions (424K pdf), which you may download from the NorQuest College website.
  • You will have to pay for this service. During this visit, the nurse can advise you of the number of additional visits and costs to complete all the immunizations you need.

If you live outside Edmonton:

  • You can receive the vaccines required and have the NorQuest immunization form completed at your local health unit. 
  • You may have to pay for this service.
  1. Submit your completed immunization record to NorQuest Health Services.

Once your immunizations are completed and they have been signed off by a nurse from the immunization business unit in Edmonton or your local public health clinic, you can submit them to NorQuest Health Services.

  1. Have your immunization form assessed by a nurse at NorQuest.

A nurse at NorQuest will review the immunization form to make sure all immunizations have been completed. If all requirements are met, the Office of the Registrar will be advised and this part of your application for admission will be complete. If you are waiting for chest X-ray results or bloodwork results in the mail, it is your responsibility to submit these records to NorQuest Health Services as soon as you receive them.

Returning Students and Immunization Records

If you have previously attended NorQuest and submitted immunizations, you need to contact NorQuest Health Services to confirm that the immunizations are still valid.  Alberta Health Services requires a TB test to be completed within six months of your program start date, and chest X-rays are valid only if they are completed within one year of your program start date.

Police Information Checks

Certain programs require a clear Police Information Check (PIC) if you are going to participate in a clinical or work placement. This document forms part of your official student record.

If your program requires a clear Police Information Check, you must obtain it before you can be enrolled in your practicum/work experience. A PIC must include a vulnerable sector check. Any fees required to get a police information check are your responsibility.

  • Edmonton residents can apply for a PIC in person through Edmonton Police Services, Police Information Check Section located at the Nexus Business Park, 108-14315 118 Avenue, Edmonton.

  • Applicants living outside of Edmonton can apply for a PIC through their local branch of the RCMP.

  • A clear information check is required before you can be enrolled in your practicum/work experience. If your PIC is not clear, you are required to meet with the Program Chair to discuss your options.

  • Police information checks do expire, so in order to make sure that it will remain valid by the time you attend your clinical/work placement, do not request it too early. Do, however, ensure that you request it in enough time to submit it by your due date. Edmonton Police Service and/or the RCMP can take anywhere from 4 to 12 weeks to complete your record check. Internet (online) police information checks are not accepted by the College. Obtain one directly from a local law enforcement agency.

  • The original police information check must be submitted, and it is then placed on your official record at the College. We do not return the original to you, nor will we photocopy it for you. If you require a copy for your own records, please ensure that you make a copy before submitting the original to us. Certain placements may require you to provide the original prior to commencement. In this instance, you will not be required to leave the original with your student file.

Intervention Record Check

If you are a student in the Social Work program, an Alberta Children and Youth Services Intervention Record Check must be submitted to the Faculty of Health and Community Studies.

  • The intervention check must meet the criteria specified by the practicum sponsors. At the appropriate time, most likely the term prior to your placement, your practicum faculty advisor will discuss your placement and what is required.
  • The intervention check cannot be dated any earlier than six months before your practicum, so do not obtain this document until advised to do so.
  • You can apply for this document through Edmonton and Area Child and Family Services Authority located at 1100-10055 106 Street NW, Edmonton (inside the HSBC building).

Workplace Hazardous Materials Information Sheets (WHMIS)

Some programs require you to have completed the WHMIS course. The WHMIS non-credit course is offered by NorQuest College Continuing Education as an online course.
 
To register for the WHMIS course:

  1. Go to the (WHMIS) Workplace Hazardous Material Information System (XBUS1017) on this website and complete the registration.
  2. Register for the course by the 15th of the month previous to the month you want to begin the course. (For example, if you would like to start the course on September 1, you must register by August 15).
  3. Complete the online course. All WHMIS courses start on the first day of every month and end on the 7th day. You have 7 days to complete the online course.
  4. Once you have completed the course, pick up your WHMIS card 3 business days after the 7th of each month from room A223 located on the second floor of the main building (10215 108 Street, Edmonton). You must provide your student ID when picking up your card. If you are a distance student, email us at continuingeducation@norquest.ca asking to have your WHMIS card mailed directly to you.

Standard First Aid/CPR

Some programs require you to have Basic First Aid/CPR certification. NorQuest College offers these certification course through our Continuing Education area. Please see Standard First Aid and CPR (XBUS1015) for more information and to register for this course. 

Human Blood-borne Pathogen Exposure

Some programs require you to have Human Blood-borne Pathogen Exposure (HBPE) training. NorQuest College offers this course through our Continuing Education area.
 
To register for the HBPE course:

  1. Go to Human Bloodborne Pathogen Exposure (XHLT1023) for more information and to register for this course.
  2. Register for the course by the 15th of the month previous to the month you want to begin the course. (For example, if you would like to start the course on September 1, you must register by August 15).
  3. Complete the online course. All HBPE courses start on the first day of every month and end on the 7th day. You have 7 days to complete the online course.
  4. Once you have completed the course, pick up your HBPE card 3 business days after the 7th of each month from room A223 located on the second floor of the main building (10215 108 Street, Edmonton). You must provide your student ID when picking up your card. If you are a distance student, email us at continuingeducation@norquest.ca asking to have your HBPE card mailed directly to you.
